Conversion Coverage

Conversion coverage may be offered by your employer if you lose your group coverage plan. Coverage starts immediately with conversion plans — in fact, it's retroactive to the date you lost prior coverage.

Did you recently lose Highmark coverage from your employer?

Individuals or families losing group coverage from a Highmark group plan may be eligible for a conversion product.

These plans provide retroactive coverage to the date your group plan ended.

IMPORTANT: You have a limited amount of time to apply for a conversion plan, or you will lose your right to this coverage.

Where am I eligible?

For enrollees who live in Allegheny, Armstrong, Beaver, Bedford, Blair, Butler, Cambria, Cameron, Centre*, Clarion, Clearfield, Crawford, Elk, Erie, Fayette, Forest, Greene, Somerset, Huntingdon, Indiana, Jefferson, Lawrence, McKean, Mercer, Potter, Venango, Warren, Washington, and Westmoreland counties (*Note: you must reside in one of the following zip codes in Centre county to enroll in one of these plans: 16677, 16686, 16829, 16845, 16859, 16866, 16874). This is a Preferred Provider Organization (PPO) plan. A PPO health care plan provides in-network benefits across a very broad network of providers, and for emergency care even when care is received from out-of-network providers.

Did you recently lose coverage from an insurer other than Highmark?

You may qualify for an individual conversion plan from Highmark thanks to the Health Insurance Portability Act (HIPAA).

To be HIPAA-eligible:

  • You must have had previous coverage through a group, government, or church health plan.
  • You must have used up any COBRA or state continuation coverage.
  • You must not be eligible for Medicare or Medicaid.
  • You must not have other health insurance.

Your window for applying for HIPAA conversion coverage lasts 63 days from the end date of your previous coverage. Your HIPAA coverage starts retroactive to that end date.

To enroll in Highmark HIPAA conversion coverage, you must complete:

  • An application.
  • A HIPAA eligibility checklist.
  • A HIPAA Prior Coverage Disclosure and Authorization.
