Healthy Transitions Meal Delivery

You're covered to receive free meal delivery to your home if you've been discharged from:

An inpatient hospital
A rehab facility
A skilled nursing facility

Once discharged from the facility, a case manager will reach out to you to coordinate the meal plan and delivery. This benefit covers up to 14 meals over seven days.

Hearing

Highmark Health Options D-SNP hearing benefits are provided by TruHearing. As a member, you’ll get a Medicare-covered hearing exam with your D-SNP. Extra plan benefits also cover:

Routine hearing exam - 1 exam each year
Routine hearing aids - 2 hearing aids (1 per ear) every 3 years

SSBCI benefits

For Our Highmark Health Options Duals Plan Members in 2027:

Depending on your eligibility, your Duals plan includes monthly benefits to help you stay healthy.
If you qualify for Special Supplemental Benefits for Chronically Ill* (SSBCI): You'll receive $212 each month. You can use this on things such as over-the-counter medicine, home and bathroom safety items, healthy food, utilities, gas at the pump, and everyday essentials. Be sure to use your funds by the end of the month because any money you don't use won't roll over.

For Our Highmark Health Options Duals Select Plan Members in 2027:

Your Select plan also offers benefits each quarter — every three months — to help support you.
If you qualify for SSBCI: You'll get $198 every three months to use on over-the-counter medicines, home and bathroom safety items, healthy food, and utilities. Be sure to use your funds within the three-month period, as they don't roll over.
